TOPIC: Customer Satisfaction Survey

Guys, let me know what you think. I put a customer survey on my site and please look it over. Do I have enough questions? Is there one that shouldn't be there? Am I missing anything? After looking at it finished I was thinking about asking a question or two about the report. I didn't want the survey too long.

Use Hyphen's for the URL rather than spaces.

Name should be a text field instead of a text area (takes up less space).

How about adding: Did you Home Inspector Smell Okay?

Kidding aside, I think it looks good!

What about using captcha ? so you don't get bots sending you messages like someone from Mars is trying to contact you.

I would take out the colons at the end of the questions and you forgot the "d" in knowledgeable and take out the extra "n" in anonymous

I would also put in a question about how they found you.
