Just type in the entire website address to the form in one of the HIP documents. It'll automatically become a clickable link in the report. You can say something like: Please fill out a satisfaction survey at: ....

Dominic, I tried to find answers here on mb before I ask, but I found a couple customer testimonials on my website, and I cannot find how to approve them. Publish is enabled, but they do not appear when I log out.

You have to copy them and paste onto your testimonial page.

okay, that works. Now, is there a way to be notified when someone comments on a page, whether it is client testimonial or blog, or another page that comments are allowed? I found a few testimonials by accident, and also had a fellow HIPster leave a comment on one of my blogs. Do I just need to check for comments regularly so I can approve/publish them?

Not at this time. Just scan it from time to time. You could make a webform instead of using the comments section and that will email you and then you can manually add the comments.
