Here's a line you may want to adopt & adapt:

Possible organic substance noted.  The United States Environmental Protection Association (EPA) states, “If you believe that you may have a hidden mold problem, consider hiring a professional.” (Brief Guide to Mold, p.14, EPA).  If any area of the residence is suspected of having organic growth, or any member of your family or household is sensitive to mold, we are lab-certified to conduct a Mold Inspection / Sampling to identify the types of mold (or any other airborne allergens) present.  If necessary, we are also lab-certified to perform radon, meth, asbestos and other environmental testing services.  See: www.IdahoHomeInspectors.biz/rates for more information.

Wow Bill, this looks great! Thank you so much for sharing it, I'm sure it'll be great for the other guys.

One thing I did notice.  You use the word Selection as a name twice within the same section, Interior Main Floor->Sample & Moisture Selection under your Location->Interior section. Because they have the same name the selection panels will be linked and always contain the same comments (though you can select separately). Did you do this on purpose so the names are the same (useful if it's the same basic list of locations). If not you can rename the sections in the template editor then make a copy of the page and it will disconnect them.

Russ, I have a similar line in my main report and the side of my van states clearly I am the MR Mold for my area, actually I am the only one doing inspection/investagations for mold, but this is what I report the mold inspection with, I am the specialist being called in to look at the mold substance, no point beating around the bush, thats why i include the sample locations and state where they were collect to also back up my COC, chain of command.
I am looking for the inspection places or items I need to add.
Also in recomendations I include the levels of remeadiation required.

With out cutting and pasting a crap load of verbage from other I am hoping to make this a HIP only addition for us to have and share.
try it if your doing mold inspections, and lets make it better.
I am looking to add a section on meth/grow ops as well to the template.

That looks great Bill!  And Russ has a great suggestion - especially if you are the only guy in the area doing this...

I just read Dominic's comment and did not know you were in Canada.  It is good verbiage however, and you may be able to adapt something like it, dropping the US stuff, and use it that way...
