The 3 states we work in all use that form. Good to know you are going to work on this! Do you have any idea of the time frame before you would have it available. It's been a while since I have made any big changes to my template or created a new one. If my memory serves, if I scan this document into HIP. I cant alter ( add the necessary data) it when the report template is open, is that correct?
