Is there a way to insert a spreadsheet or a calculating page into a web form (or using a web form entry) to calculate inspection fee?

Is there a home inspector web form available that will allow a client to enter "numerous inputs" ie, sq ft, age of property, etc, which will allow an automatic quote estimate to be generated.
