TOPIC: State of Florida - Citizens Insurance reports

Is there a way to implement the Citizens four point report into HIP to complete the form and add pictures?
Attached is a copy of the Citizens form. They are getting more and more necessary now that carriers are falling out of Florida and the few that are here are asking for these more often now.
