TOPIC: insurance - four point

In Florida more and more insurance companies are needing citizens four points completed. Its getting so bad that they are starting to require them on homes as old as 10 years. Do you have a citizens four point form we can use to fill in similar to the wind mitigation? Citizens will only accept there form completed not the citizens report thats on the new version of HIP but the citizens form. If not do I need to search or do you recommend another company that has the form?
