TOPIC: Check Boxes

Where did they go ???? I see them appear and empty in my pdf report but not to be found now when I am filling out the report.
I can see how to edit them or remove them but nothing to make them appear in each section.
Help ??

Click Edit Comments along the top of the program to Toggle between edit and rating mode.
