Hi Joseph,

The default template is more than enough as they use the ASHI SOP which several other licensed states use. But you can contact some of the other guys.

Mark Timpani posted a sample report a year or two ago from Phoenix and there's lots of AZ guys that post here. You can check out his sample here: www.homeinspectorpro.com/component/optio...ic,73.msg359/#msg359 and contact him as well.
