Roy just saw this thread and after clicking the first sample at first thought it was cool but fading to black on a report ?.....no way.
second thing I saw and stopped here on fast scroll through was this.....6.1. The kitchen ceiling light was inoperative...Bulbs?

You asking the client if it was the bulbs ?.....that's why they pay you dude.

Just being hard on you Roy but seriously reword that.
Might be better to say not functioning .
The second example I stopped when I saw it had the same Black fade out.
Bad design  but cool artistically.Trust me I could get creative myself but hold back due to it is a bad business mix.

Very nice report, I am not a fan of the stationary as the darker areas make it harder to read. I like the orange and your usage of the glossary.
