I read this thread, then went to the survey.
Here is my honest opinion...
I HATE surveys where I fill in the blank. I will not do them. White boxes or not, you will get very few QUALITY responses. Make them multiple choice, and add a box IF THEY WISH to elaborate. Make a statement that the boxes are optional but very much appreciated. Let them know it is their CHOICE and NOT required.

Here is something a previous boss, in another life, once told me... "Make it easy for the client to say YES"!!! Think about it, it applies to most everything when we deal with clients/customers.

Thanx for the suggestions guys. I am going to shorten it. Jeff, just having yes or no questions defeats my purpose of getting positive comments to use on my page for client comments.

I understand that Joe, but what good is a survey that no-one fills out? If done properly, it will give you the info you want, without losing the client to giving honest feedback. Have you considered multiple choice with the option for elaboration?
